Well, we have a testing team who are accountable for finding defects.
Wouldn't trust the developers.

A bit foolish upgrading to a new operating system without backing up all
your data!

Not trying to defend Apple or anything but users have to share a bit of the
responsibility.

Swot I was thinking. If my place only released stuff with zero known
defects, nothing would ever get out the door.

Surely it's accepted that some bugs are not serious enough to hold up
a release? Any mature software delivery function makes these sorts of
decisions all the time.
